The Neck and Jawline After GLP-1 Weight Loss: What Changes and Why

Most of the conversation about facial changes during rapid weight loss focuses on cheeks and temples. But the complaint I hear most often from women over 45 is about the neck, and it usually arrives with a particular kind of frustration: I finally lost the weight, and now this. Let me explain what is happening below the jawline, because the mechanics are slightly different from the face and the fixes are not identical either.
Why the Neck Behaves Differently
The skin on your neck is thinner than the skin on your face. It has fewer oil glands, less structural support, and it spends its life folding and extending every time you look down at a phone. It was already the least forgiving skin you own before anything changed.
Now remove the fat underneath it quickly. The face has distinct fat compartments that deflate in a fairly predictable pattern. The neck and the area under the jaw hold a more diffuse fat pad, and when that shrinks, the skin envelope that was draped over it has nowhere to go. It does not shrink at the same rate. That mismatch is what produces the loose, crepey appearance and the softening of what used to be a defined jawline.
What the Research Tells Us
The imaging work on this has focused on the midface, where a 2025 study documented roughly 9 percent total volume loss, concentrated in the superficial compartments closer to the skin surface1. That superficial concentration is the relevant finding for the neck and jaw, because the changes people notice along the jawline are largely about what happens near the surface rather than deep against bone.

The Age and Menopause Factor
I want to name something directly, because it explains why two women losing the same 40 pounds can have completely different experiences. Collagen and elastin production declines with age and drops off more sharply after menopause. Skin that has less collagen to begin with has less capacity to retract. If there is one lever with real leverage here, this is it. Skin remodels slowly. A slower rate of loss gives the skin envelope time to contract along with the tissue underneath it. This is a conversation to have with your prescribing physician about titration, and it is a legitimate reason to raise, not a vanity concern to apologize for.
Protein, Because Skin Is Built From It
Skin is protein, and it is not high on the body's priority list during a deficit. Guidance for adults losing weight on these medications lands at 1.2 to 1.6 grams of protein per kilogram of body weight daily4. Falling short does not cause loose skin, but it removes the raw material your skin needs to maintain and repair itself while it is under strain.
Collagen, With Realistic Expectations
The evidence here is genuinely mixed and I will not oversell it. A 2023 meta-analysis of 26 randomized trials found improvements in skin hydration and elasticity with hydrolyzed collagen5. A 2025 meta-analysis stratifying trials by funding source found that studies without industry funding did not show a significant effect6. Research suggests collagen peptides may support skin hydration and elasticity, results vary, and eight to twelve weeks is a fair trial period. What collagen will not do is tighten skin that has lost its underlying support. Nothing you drink will.
What Is Outside Nutrition
I would rather be honest than sell you something that cannot deliver. Significant skin laxity along the jaw and neck, once established, is generally addressed with procedures rather than nutrition. If it is affecting you meaningfully, a board-certified dermatologist or plastic surgeon can walk you through the options. That is not a failure of your diet. It is a structural problem that needs a structural answer.
Keeping Perspective
Your neck did not age five years in six months. What happened is that support was removed faster than the skin could follow, and some of that settles as weight stabilizes and the pace slows.
